The Call Center is available at (317) 944-7744 between 8:00 a.m.- 4:30 p.m. Monday through Friday. 

When making an appointment, we will ask you: 

You will receive an automated courtesy call and a packet of information and forms prior to your appointment. To assist in the registration process, please bring the completed forms with you to your appointment.  If you need to reschedule or cancel your appointment, please contact the Call Center.

On the day of your appointment:

Arrive at least 15 minutes prior to your scheduled appointment. Each site has parking available but take that into consideration so you arrive on time at the check-in desk.

Bring your insurance card and a photo ID.

Bring your co-pay. If your insurance coverage includes this, it is due at time of visit. We accept cash, check, Visa or MC.  Services not covered under insurance may require full payment at time of service.

You will complete a patient information form (for new patients and patients not seen within the previous year). This form may be downloaded  medical history form and completed prior to your appointment. 

Bring a list of medications. We recommend you bring all medications you have been prescribed for your skin condition so we know exactly what you have tried before.

If you have been evaluated by other dermatologists, please bring their notes including any biopsy reports. 

We request 24 hour notice to cancel or reschedule your appointment. Please call (317) 944-7744, during business hours. At other times, please leave a voicemail message.
